Well, I’m 60 now and getting a lot of questions about how much longer I plan on working, so let’s discuss!  Turning 60 is a milestone in life, and for some people, it may signify the nearing end of their working career. However, for others, it may be just the beginning of a new chapter in their lives. For those who have a passion for their work, retirement may not be an option they want to pursue. I fit squarely in this camp.

 

The business of dental practice is not just a profession for some dentists, it is a calling, and for me, the opportunity to help people maintain their oral health and improve their smile is immensely rewarding. The dental industry is constantly evolving, and there is always something new to learn, making it an exciting and stimulating field to work in. Currently in our practice, we are pursuing the emerging use of AI to enhance interpretation of xrays, biotechnology to enhance healing associated with bone grafting, and computer-assisted navigation for implant placement. For a gadget geek, this is fun stuff!

 

Clinical dentistry is a profession that requires a high level of skill and expertise, which can take years to acquire. Many dentists who have spent decades perfecting their craft may not want to put it on the shelf. And, it’s not just the technical skills that keep dental practitioners engaged; it is the relationships they have built with their patients. These relationships are often built over several years, and dentists may feel a sense of responsibility towards their patients. I know I do.

 

In conclusion, my work is not work for me; it’s actually my number one hobby. I have other hobbies—many of you are aware I am a musician—but none offer the thrills and chills of running a large dental practice! I come in every day and engage with an amazing staff, amazing patients and amazing technology. I use my hands and mind to create oral health solutions for a wide range of conditions and a wide range of personalities, and it is my goal to continue to do this at a high level for another 12 – 15 years. I invite each of you to continue with me on this fun journey. So, let’s get going!

With only days left to finalize your Christmas shopping, the pressure is on to find just the right gifts! We have some great, last-minute stocking stuffer ideas, perfect for anyone on your list. With all the added indulgences this time of year, it’s important to give your teeth a little extra love and attention.

 

Toothbrushes tend to be an item that are used well past their recommended life expectancy. Christmas is a great time to purchase your loved ones a new one… and, they are just the right “stocking-stuffer” size! There are many manual or electric options to choose from depending on their likes or needs or …your budget.

TMJ, or Temporomandibular Joints, connect the jawbone to the temporal bone on both sides of skull. These joints act like a hinge to assist the jaw with movements such as chewing or talking.

TMD refers to Temporomandibular Disorders. TMJ disorders are very common. They range in terms of severity and occur when the muscles surrounding the joints have experienced some sort of trauma.

Dentists are oftentimes the first to witness and diagnose TMJ disorders due to their knowledge of the jaw. Being that November is TMJ Awareness Month, we wanted to share some insight into this prevalent and unfortunate condition.

 


Common Signs or Symptoms of TMJ Disorders – What to Look For:


 

  • Pain in the jaw area
  • Headache
  • Pain while opening the mouth or while chewing
  • Pain or ringing in the ears
  • A clicking, popping or grinding sound upon opening or closing the mouth
  • Locking of the jaw in the open or closed position

 

TMJ disorders can be caused from one or several factors making an exact origin sometimes hard to pinpoint. They are usually a result of trauma but may also be linked to:

  • Bruxism – or clenching/grinding your teeth
  • Misaligned bite
  • Arthritis
  • Stress

 


What Can I do to Help My TMJ:


If you’re currently experiencing any of the above symptoms, it’s best to get an evaluation. Your dentist will do an examination to look for signs of clenching or grinding and may ask to observe as you open/close your mouth. X-rays or additional scans may also be needed.

Treatment options vary depending on the cause and severity of your TMJ disorder and may include some combination of the following:

  • Wearing a night guard to prevent from clenching and grinding
  • Referral to a physical therapist to learn a variety of exercises that help reduce pain and regain normal movement
  • Education on changing daily habits that may aggravate TMJ
  • Acupuncture or adopting stress-reducing behaviors such as meditation
  • Non-surgical procedures such as Botox injections or Ultrasound
  • Surgical procedures

TMJ pain can worsen and lead to more serious conditions. Essentially, this software gives our doctors a “second set of eyes.” Immediately upon taking X-rays, Pearl has the capability of diagnosing nine different diseases and/or dental conditions.

 

For example, it can detect decay in its earliest stages and abscesses before they become painful or problematic. It lets the doctors know if a root canal is recommended, if periodontal disease is present or even if the margins of existing restorations need attention. It’s a very exciting time for dental hygienists in North Carolina! Governor Roy Cooper recently signed a law enabling our state’s dental hygienists to administer local anesthesia to dental patients. This is a huge win for our dental hygienists being that North Carolina was one of only 5 states still restricting this practice. This certification is acknowledged by the North Carolina State Board of Dental Examiners and upon completion, enables hygienists greater autonomy in the scope of dentistry and will also lessen some of the demands placed on our dentists.

 

 

 

 

 

 

To earn this certification, our hygienists must complete rigorous coursework including 16 hours of online class time and tests as well as supervised in-person practical experience on live patients. Our hygienists have already been participating in online coursework and have their in-person practical April 2nd.

 

In addition to the stringent North Carolina state requirements, Dr. Gillespie has developed a pre-certification program wherein our hygienists are slowly introduced into the actual administration of local anesthesia. Initially, this involves practicing on avocados, oranges and chicken thighs. Avocados have a tough skin with a pit, mimicking what it’s like to find bone with the needle. Oranges have a tough, thick skin allowing them to practice dispensing an entire carpule of anesthetic and chicken thighs mimic the lower jaw with tissue supported by bone.

As our hygienists gain proficiency, we have several willing in-office volunteers who will be anesthetized under supervision. After certification, Dr. Gillespie and our other dentists will supervise anesthetic administration to these helpers in the effort to make this transition seamlessly!

Not only is this a huge milestone for North Carolina and all the hygienists currently licensed in our state,  allowing our clinicians this sovereignty also benefits dental practices and YOU, our patients!

  • No waiting on dentists – dentists maintain a busy restorative schedule & this will keep them from having to leave their patient schedule simply to administer local anesthesia for a periodontal cleaning

I always knew Matt (nèe Matthew to Mom & Dad), would study business. I knew it from the first week of first grade. That was the week he came home with almost ten dollars from selling pencil grips for a dollar each to his classmates, most of whom he did not know before school started. Betsy had paid two dollars for a pack of ten and Matt took them to school and sold them all save one, which he kept back for himself. His cost basis in this venture was zero, so he was earning pure profit and learning Business 101 all by himself at the tender age of six.

In second grade, we stopped Matt on his way out the door with an overpacked book bag.  When asked about it, we found out he had been selling some of his inventory of stuffed animals for five dollars each to his classmates. We asked him to let us see in the backpack and when he opened it up, a rather large stuffed lion leapt out of the bag, which he said he was selling for ten dollars. Be still my heart.

You just can’t teach some things, and while go get ‘um is something that can certainly be honed, I think mostly it’s innate.  And Matt has it in spades. He graduated from NC State this past weekend with a BSBA degree with a Marketing concentration as well as a minor in Spanish and starts his first job as a newly-minted marketer with a firm in Raleigh in just a couple of weeks. He’s already thinking about buying his first home to develop as a rental and give him a second income stream right out of the gate. I’m one proud dad.  Proud indeed.  Congratulations, Matthew!

You may have heard that this  year is our office’s 25th Anniversary!  We are so proud of this and plan on celebrating throughout the year- and you know how we love to celebrate!  But first and foremost, we want to honor the relationships and intentions that built this practice.  Those start with you, our patients.  Whether you’ve been with us for a week or for a few decades- we are incredibly grateful!  We can’t say enough about how much joy we take in helping you live your healthiest, best life.  

Over the next few months, we’ll be releasing interview sessions with Dr. Gillespie that illuminate our practice’s philosophy on patient care.  Please take a moment to hear our hopes for you and celebrate these 25 years with us!   

One other special occasion coming up is Mountain Xpress’ Best of WNC voting for the 2018 year!  For those of you that have voted for us in the past, thank you!  We are going on  our 8th year running as Best Dentist and would love your continued support.  Please take the time to vote for us in the BEST DENTIST category at Best of WNC 2018  !
